Getting Ready For Your Appointment

New patients are advised to show up at least 30 minutes before your appointment time to allow enough time to register, sign paperwork, verify insurance coverage, and sign up for the patient portal. Existing patients are advised to show up at least 15 minutes before your appointment time to validate existing patient information, verify insurance coverage, and sign paperwork. You will have the option to receive an appointment reminder via phone, text or email. Please remember to confirm your appointment when notified.

Want to save time? New patients may download and complete all forms contained in the Patient Registration Packet prior to the appointment.

All patients should bring the following items: any applicable forms, driver’s license or proof of identification, current health insurance card, current medications. To determine if you are eligible for a discount on services bring additional items: proof of income and proof of address.

Most private insurance plans are accepted. Call for verification. We also accept AETNA, Ambetter, Amerigroup, BCBSTX, CHIP, CIGNA Healthspring, Cook Children’s Health Plan, Delta Dental, Humana, Magnum, Medicare, Medicaid, MetLife, Molina, Parkland, Superior, TriWest-VA, and United Health Care (UHC).

Sliding Fee Discounts

Sliding fee discounts are available to those who qualify. Specific conditions must be met, and all appropriate documents must be submitted. Sliding fee calculations are based on income and size of household. If you do not qualify for the sliding fee discount or do not have all documentation, you will be responsible for paying full price. We accept cash, check and credit cards.
